McChord Optometry is in network with Tricare, Medicare, Aetna, Premera, Regence, Lifewise, Uniform Medical, Kaiser Permanente’s PPO plans, Lifewise, Blue Cross/Blue Shield, First Choice Health, Davis Vision, Eye Quest and March Vision plans. Your vision insurance carrier may be different than your medical insurance carrier. We do not accept some vision plans such as VSP or Spectera due to unreasonable contractual requirements. If you are paying for a Glasses Eye Exam out of pocket the cost is $85; additional fees apply for contact lens fittings.

In order to help you prepare for your appointment, it would be great if you would review the documents below. If possible, please print & complete our patient intake form and bring it to your appointment. Otherwise please arrive 5 minutes prior to your appointment time to fill out the paperwork in office.

If you need to request a copy of your medical records be sent to (or from) us, please complete the Medical Information Release Form and return it to the doctor’s office.
